You can get these records from your pediatrician, high school or from a previous college you attended. If you are unable to get these records, you can have blood work drawn for MMR titers and submit the lab report showing immunity.

Yes, you may have your immunizations and health record completed at the Health and Wellness Center. There is a fee for the service and an appointment is required. Please call 934-420-2009 for an appointment.

We prefer you make an appointment, but if you feel sick and it is urgent, you can always come in during the day and be seen by one of the nurses. Please call for an appointment for the doctor or nurse practitioner since they are not always in the office.
